Welcome to the Larrabee Center for Plastic Surgery

Below you will find five pre-registration forms, including a patient registration form, a health questionnaire, a privacy practice acknowledgment form, a cancellation policy, and a photo authorization form. Please complete all five forms and bring them with you on the day of your appointment. This will help us in expediting your check-in.

Please be aware that there is a $150 consultation fee that applies to your service when booked. This is payable at the time of your initial visit with our physicians.  If you are unable to keep your appointment, please notify us 48 hours in advance. We do charge a cancellation fee for missed appointments without prior notification.

The physicians and the support staff at the Larrabee Center want you to feel comfortable, educated, and well-cared for in regard to your surgical and aesthetic decision-making.
